Noble Surgery

The facial features of Asians may be relatively flat, making it difficult to highlight a noble temperament. One method, informally referred to as "Noble Surgery," aims to address this. This procedure involves the implantation of implants through the oral cavity to elevate the areas around the nasal wings and the central part of the face that may be concave, creating a more aristocratic facial contour. The surgery is performed under painless sleep anesthesia and patients can immediately resume their normal daily activities postoperatively. Since the surgery is conducted inside the oral cavity, there are no visible external signs, making it a relatively straightforward procedure. This surgery is often combined with rhinoplasty. In some cases, undergoing only this procedure is sufficient to improve issues such as smile lines and lateral facial depressions, resulting in a more youthful and elegant appearance. The scope of the surgery can range from the concave areas around the nasal wings to the area of under-eye wrinkles, and even include the entire region of the cheeks and smile lines.

Surgical Methods

Filler (Hyaluronic Acid)

The use of fillers such as collagen, Restylane and Artecoll injected around the nostrils to increase elevation is a simple and popular method. This approach provides immediate results. However, the effects of fillers are relatively short-lived, requiring periodic procedures to maintain the results, which can be inconvenient.
For younger individuals or those with mild laugh lines, fillers like Restylane can be used effectively. However, if there is a more significant depression beneath the entire nose, surgical correction may be necessary for precise and long-lasting results.
When deciding on the appropriate method, it is advisable to consult with a doctor for a detailed assessment and discussion to develop the most suitable treatment plan based on individual circumstances.

Implantation Of Prosthesis

The surgical method involves placing the prosthesis in the anterior part of the maxillary bone and the side part of the nose to achieve elevation in the depressed area. This surgery usually does not require hospitalization. During the procedure, the doctor will make small incisions through the mucosa inside the mouth and place artificial implants such as silicone, fillers or a combination of materials above the bone beneath the lower part of the nose to lift the depressed area. At Beautyline4U hospital, we perform dissection below the mucosa through the incision, fix the substitute bone or implant in the bone beneath the depressed area. This allows blood vessels and surrounding tissues to naturally grow into the internal space of the implant.
Over time, the surgical scars will gradually fade, making it difficult for both the patient and the operating doctor to perceive the presence of the surgery. The facial contour and shape will become natural. The foreign body sensation of the prosthesis will also gradually disappear, and there will be no unnatural feeling when the lips move. The implants used in aristocratic surgery such as silicone or fillers, help support the tissues beneath the nose and maintain a tight feel, making them widely used.
The surgical incision is made inside the oral cavity or nose, and its length is less than 1 centimeter, so there is no need to worry about scarring. Placing the prosthesis under the periosteum and suturing through the incision is a relatively simple surgical method.

Autologous Fat Transfer

The method of extracting surplus fat from the abdomen or thighs and injecting it into the area extending from under the nasal wings to the edge of the lips is a simple and fast-recovery option. While this method can improve the concave areas beside the nasal wings, its effectiveness is relatively lower compared to fillers or implants. Since it involves autologous tissue transplantation, there are no side effects, and it is also a good method for correcting the nasolabial folds around the mouth. As some of the transplanted fat may be absorbed, a 2nd transplantation may be necessary to achieve the desired results.

Side Effects

After the surgery, there may be a foreign body sensation around the lips. This sensation is typically most noticeable around 2 weeks after the surgery and gradually diminishes as patients adapt. Numbness is common after the surgery, but this abnormal sensation will gradually fade over time and does not lead to any issues.

Recovery Time

After the noble surgery, there may be slight swelling. Since the operation is performed inside the oral cavity, the advantage of this method is that it does not leave any linear scars. However, it is necessary to consider the possibility of swelling of the lips and mucous membranes. About 2 days later, only the lips may swell slightly, but it will not affect daily life.
